    Teacher, 28, died from untreated blood clot

    Dena Collins, a 28-year-old primary school teacher, died from a heart attack caused by a blood clot after being discharged from Queen Elizabeth Hospital. A coroner's inquest revealed that had the blood clot been diagnosed and treated, her death could have been avoided. Collins had visited her GP and hospital multiple times before her fatal collapse.
