    Police did not arrest a child abuser despite a warrant issued 18 months prior to the murder of two-year-old Isabelle Welsh. She suffered severe abuse, including 21 broken bones, before her death in September. Her mother and her mother's boyfriend were convicted of murder and child cruelty. Police admitted failures in executing the arrest warrant.
