    On June 30, 1971, three Soviet cosmonauts—Georgi Dobrovolsky, Vladislav Volkov, and Viktor Patsayev—died during re-entry from the Salyut 1 space station due to sudden depressurization. Despite a flawless landing in Kazakhstan, the crew was found dead from asphyxiation. The incident was linked to a malfunction of a pressure equalization valve.
